Baptism Sponsor Form
Together with the parents, the sponsor or godparents presents the child at baptism and helps this child to lead a Christian life in harmony with baptism and to fulfill faithfully the obligations connected with it.
To be a Sponsor for a Baptism at another parish you will need a Certificate of
Sponsorship.
The requirements to receive the Certificate as follows:
• You have received the three sacraments of initiation: Baptism, Confirmation, and
the Holy Eucharist.
• You are a sincere believer that Jesus Christ is your Lord and Savior.
• You are a registered member of St. Patrick Parish and a regular active
participant in Sunday Eucharist.
• You are dedicated to living your life according to Gospel values.

Sacrament of Baptism above the dotted lines and return to the Parish Office
